We ensure the quality of care through our Registered Nurses, Licensed practical Nurses, Occupational Therapist, Activities Director, Assistants and Shuttle Drivers. We carefully selected our employees, in caring for "Our" Seniors and your loved ones. Home and community based service program (hcbw) "medicaid waiver"

What is it?

  • Medicaid waiver is a special Medicaid program for the elderly and disabled that will pay for certain home health services not otherwise covered by Medicaid or Medicare

What is its purpose?

Its purpose is to allow a person to remain at home rather than entering along-term care facility. Some services include:

  • Adult Day Care (ADC). The patient can be enrolled in the waiver program and attend the ADC of their choice.
  • Light assistance with homemaking, such as dusting and vacuuming of the patient's area, doing the patient's personal laundry, errands, grocery shopping, and light meal preparation.
  • Personal Care, such as bathing and grooming.
  • Respite for caregiver relief ( up yo 80 hours every 6 month)
  • Case Management by a qualified professional RN or Social Worker.

How does someone qualify?

The Patient qualifies in two ways. The level of care criteria must be met and the criteria to qualify for Medicaid must also be met. Our qualified staff will be able to discuss the application process with you and point you in the right direction.
